California citizens and quality oriented, accurate and reliable clinical
laboratory testing. LFS provides oversight for clinical and public health
laboratory operations and for the licensed and certified scientists and other

and Clinical Laboratory Bioanalyst/Clinicial Laboratory Director licensure and
Phlebotomy certification may now be submitted electronically online to the State of California
Department of Health Services.
NOTE:
Applicants
who have already submitted a paper application do not need to re-submit an
application using the online process. LFS will continue to process all manual
applications received. Duplicate submissions may result in delay of the

Issues Unique to California Law
Laboratory Field Services of the California Department of Health Services has
been concerned for a number of years about whether state law issues were being
addressed during surveys by laboratory accrediting organizations. In order to
help accredited laboratories, and all laboratories, better understand how state
law differs from federal (CLIA) law, Laboratory Field Services has prepared a
list of state law issues unique to California. This listing, dated May 2003, may
be used as a guideline to assure that your laboratory complies with state law.
Laboratory Field Services plans to update this list each year to keep everybody
better informed.

Authorization to orders tests and receive results
The Department of Health Services (Department) is receiving numerous requests
for clarification about who is authorized to order clinical laboratory tests
and/or receive the test results in California. This notice is intended to
clarify the state and federal requirements.
